Course Description

This module elevates the profile of socioeconomic determinants of health and community risk factors on Latino health disparities, the role providers in the provision of culturally effective population health management, Equality Health's Socio-Cultural risk assessment and community resource platform for linking patients to community based services, and self-management of patients with complex chronic illness using information tools and mobile applications.
